 At Southwest Louisiana Center for Health Services every effort is taken to assure that our fees for services are among the lowest in the community. However, our services are not free.
SWLA Center for Health Services is a
501(c)(3) community-based organization
providing preventive and primary health
care for the entire family regardless of
their ability to pay using a sliding fee scale. We also welcome Medicare, Medicaid, private insurance and self-pay patients.
Sliding Fee is a special program at SWLA Center for Health Services
(SWLA) to assist those who are uninsured or have difficulty paying for medical
care. Depending on your income and family size, sliding fee scale decides how
much you and your family will pay for medical services. Please note that not all procedures may qualify for sliding
fee discount.
Required Proof of Income: Current pay stub, Unemployment award letter, most current W2's, SSI award letter or a copy of the check, college grant award letter and certification letter from Medical Assistance. For more information call 337-439-9983 |
