Patients will need to visit the SWLA location closest to them and fill out a Release of Information form. Please bring (1) form of identification.

No charge. Obtaining your medical records is free at SWLA Center for Health Services.

Once the release of information is signed, it takes 24 to 48 hours to receive the records. Someone from the Medical Records department will contact you, when your records are ready to be picked up.

No records can be faxed or mailed. Any patient that would like to retrieve their medical record, they must visit the SWLA location closest to them and fill out a Release of Information form. Please bring (1) form of identification.

To get medical records sent to another provider, the patient must visit the SWLA Location closest to them and fill out a release of information form. Please bring (1) form of identification.

To get your child’s shot records, telephone the SWLA call center and ask for a message to be given to the Health Information Center.
