Patients have the right to:
Appropriate assessment and management of pain
Reasonable access to care
Considerate care that represents each patient’s values and beliefs
Participate in decisions regarding his or her care
Participate in the consideration of ethical issues that arise in the provision of his/her care; including giving informed consent and resolving dilemmas about care decisions
Personal privacy and confidentiality of information
Appoint a representative to make a decision on his or her belief, in the event that the patient is incapable of understanding a proposed treatment or procedure or is unable to communicate his/her wishes regarding care.
Informed of these issues related to his/her care
Have providers who are educated by the center about patient rights and their role in supporting those rights
Access to protective services


Patient Responsibilities
You are responsible for providing accurate and complete information about your health and any matters related to your health
You are responsible for asking questions when you do not understand information or instructions
You are responsible for following the treatment plan recommended by your providers
You are responsible for keeping any appointments and to call in advance for cancellation or rescheduling
You are responsible for treating all staff members with respect and consideration
You are responsible for making payments for your care and treatment rendered by the foundation unless previous arrangements have been made
You are responsible for providing correct information necessary for insurance processing of your bill
You and your visitors are responsible for considering the needs of others; including patients, visitors and staff by maintaining noise control, not smoking on SWLA property and keeping the number of visitors in the exam room down to a minimum.
Discover Our Women’s Health Services
SWLA Center for Health Services provides Obstetrics (OB) services for women before, during and after childbirth and by referral for qualified specialists for certain cases. Our Gynecology (GYN) services provide preventative check-ups along with diagnosis and treatment of disorders affecting the female reproductive system.
