Our Patient-Centered approach ensures coordinated treatment for each patient. Your medical team could include a case worker, a nurse, a physician and/or a nurse practitioner, a health educator and a fitness trainer. This comprehensive approach will surround you with the services required to maintain good health.

​Adult Medical Services

Adults receive quality primary and preventive care services from the Adult Medicine Department. Our compassionate, culturally competent physicians and nurse practitioners are qualified to meet the medical needs of all their patients.

  • Complete physical exams including insurance physicals
  • Preventive health care and education
  • Regular physician and nurse practitioner visits
  • Lab testing 
  • Management of acute and urgent conditions
  • Screening and management of chronic conditions such as high blood pressure, asthma, cholesterol, and diabetes

  • Immunizations

  • Cancer prevention and detection
  • Nutrition and weight loss

  • Referrals for necessary outside services such as radiology, physical therapy, home health care, hospital care, specialty care, etc

Blood Pressure Control Initiative

SWLA Center for Health Services aims to educate our patients on self management, especially through our self-measure blood pressure program (SMBP).

SWLA CHS established this program to allow patients to play an active role in their care.

By entering into this program, our staff will:

  • Measure your blood pressure accurately

  • Repeat the measurement if your blood pressure is elevated

  • Prompt a remote patient monitoring referral for you

  • Provide you with educational resources and a streamlined process for blood pressure measurement and follow-up.

Using this strategy, SWLA CHS has increased blood pressure control among our patients from 44% to 69% in 2022.

Pediatrics

SWLA Center for Health Services offers family-centered pediatric services that serve our pediatric patients from birth through adolescence.  If you are expecting a baby, we encourage you to schedule a “Meet & Greet” with one of our pediatric providers. 

According to the Centers for Disease Control and Prevention (CDC), in Louisiana alone, 156,235 children do not have excellent or very good health.

Annual child wellness checkups play a crucial role in promoting your child’s well being, preventing illnesses and making sure your child is developing normally.

SWLA Center for Health Services accepts all patients regardless of their insurance or financial status. We are a federally qualified health care program which in turn allows us to offer a wide range of services to patients through the sliding-fee scale.
