SWLA Center for Health Services will be making our annual nursing home visit in Lafayette on Friday, December 16th, 2022 from 10:30 am- 11:30pm.
Location: Lady of the Oaks Retirement Manor- 1005 Eraste Landry Rd. Lafayette, LA 70506
SWLA Staff will be mixing and mingling with the residents along with handing out SWLA CHS care packages.
SWLA Center for Health Services sites are Federal Tort Claims Act (FTCA) deemed facilities. SWLA Center for Health Services is a 501 (c)3 non-profit community-based Federally Qualified Health Center (FQHC) offering comprehensive medical, dental, mental health, and specialty healthcare services. SWLA is a Health Center Program grantee under 42 U.S.C. 254b, and deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
