SWLA Center for Health Services will be making our annual nursing home visit in Lake Charles on Wednesday, December 14th, 2022 from 11:30 am- 12:30pm.
Location: Landmark of Lake Charles- 2335 Oak Park Blvd. Lake Charles, LA 70601
SWLA Staff will be mixing and mingling with the residents along with handing out SWLA CHS care packages.
SWLA Center for Health Services sites are Federal Tort Claims Act (FTCA) deemed facilities. SWLA Center for Health Services is a 501 (c)3 non-profit community-based Federally Qualified Health Center (FQHC) offering comprehensive medical, dental, mental health, and specialty healthcare services. SWLA is a Health Center Program grantee under 42 U.S.C. 254b, and deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
