Location: 500 Patterson Street Lafayette, LA 70501 NO APPOINTMENTS NEEDED! If you would like to apply for Medicaid, all you need to do is show up, with (1) form of identification, to the Lafayette site between the hours of 9:00am-12:00pm. SWLA CHS also has a few more surprises in store, during this week, to show our patients how much we continue to appreciate them!
